Membership Agreement

References to "Body," "we," "us," or "our" mean SRB Analytics Inc. d/b/a Body.

Membership access

Your membership provides access to Body studios and related services, subject to appointment availability, studio capacity, safety requirements, modality-specific spacing, and recommended testing cadence.

Founding membership

Founding membership is $499 per year and is reserved for Body's inaugural Hudson Square cohort unless otherwise stated in writing.

Wellness-only relationship with Body

Body is a wellness and performance membership. Body does not provide medical care, medical advice, diagnosis, treatment, or medical opinions. Use of Body does not create a doctor-patient relationship with Body.

Third-party services

Certain services may be ordered, reviewed, processed, or provided by independent third-party clinicians, physician networks, laboratories, imaging providers, or other healthcare entities. You may be subject to those providers' separate policies, consents, and obligations.

Follow-up responsibility

You are responsible for sharing results or concerns with your own healthcare provider and for seeking appropriate medical follow-up. Body is not a substitute for your physician or other healthcare providers.

Safety and eligibility

Body memberships and services are currently available only to individuals 18 years of age or older. You agree to follow studio instructions, safety guidance, and staff directions during each visit.
