References to "Body," "we," "us," or "our" mean SRB Analytics Inc. d/b/a Body.

When this notice applies

This notice applies only to protected health information maintained by Body when Body is acting in a role that makes HIPAA applicable to the information we handle, such as under a business associate agreement. Other entities involved in providing services may have separate notices.

Independent healthcare entities

Independent laboratories, physician networks, clinicians, imaging providers, and other healthcare entities may maintain their own HIPAA obligations, notices, policies, and patient relationships. Body does not control those separate obligations.

Permitted uses and disclosures

Where HIPAA applies, information may be used or disclosed to support treatment, payment, healthcare operations, legal compliance, member-authorized activities, and other uses permitted or required by law.

Your rights

Where HIPAA applies, you may have rights to access, amend, restrict, or receive an accounting of certain disclosures of protected health information, subject to applicable law and verification requirements.
